Cascade Public Schools is working with Alluvion Health to establish a community-based health clinic for all residents of our school district. We need your help. Please complete the survey to allow the District and Alluvion Health to determine the needs of our community. Please share this story and the link to the survey with as many community members as possible.

Who can use the School-Based Center?

Everyone! The medical clinic will be open to all staff employed and students enrolled in Cascade Public School, as well as their families. Community members in Cascade may also be able to access the School-Based Center once it is fully implemented.

Will there be a charge for the School-Based Center's services?

The School-Based Clinic will accept Medicare, Medicaid, Healthy Montana Kids, and most insurance plans. If you have insurance, Alluvion will bill directly to your insurance company. You will be responsible for co-pays and any unmet deductible amounts. For both those with or without insurance, we offer a sliding fee scale that is based on income and family size. No child will be denied services due to the inability to pay.

In order for students to receive services at the School-Based Health Center, a complete and signed consent form will need to be returned to the School-Based Health Center. The consent must be on file prior to the child being seen. Once the Center is set up, further information will be communicated, and consent forms sent out.
