To All students in grades 6th - 11th who are planning to participate in sports next school year.

We will be traveling to Great Falls on Thursday May 10th for our annual sports physicals.  The parent consent form is attached.  This form, as well as a $20 payment, needs to be turned in to Mrs. Mazaira by Tuesday, May 8th in order to organize our transportation.  Students who turn in their form after Tuesday will not be guaranteed a seat on the bus. Make checks payable to Cascade Public Schools.

Please wear appropriate clothing - preferably shorts and t-shirts.  If your child wears glasses, please make sure to have your child bring them.
