Forms
Current sports physicals and athletic forms are required before a student will be allowed to attend practice. Follow this link for detailed instructions on registering your athlete for participation in athletics /files/user/115/file/athletic%20requirements_dragonfly.pdf
The following forms are required before a student is allowed to attend practice. All forms will need to be submitted on-line through the Dragonfly website.
1. Physical Form (MHSA required form)
2. Concussion Statement (MHSA required form)
3. Assumption of Risk (Cascade form)
4. Handbook Sign-Off-Activities/Athletics Signature Page (Cascade form)
The activity fee will also need to be paid before beginning practice. This fee can be turned in to the AD or the Front Office.
